Please be sure to have all of these documents before you apply. 
 
·         Service Accountability Agreement with its LHIN
·         Integrated Health Services Plan
 
If the front-line RNs are represented by a union:
 
·         Provide a current salary grid and documentation related to any negotiated increase pertinent to the application.
·         Provide the collective agreement article with respect to the percentage in lieu of benefits pertinent to the application
 
If the front-line RNs are NOT represented by a union:
 
·         Provide information regarding salaries/wages and % in lieu of benefits
